This will trigger after collecting all hats from the various chapters of the game.
Evasive prey

From the start of the game, players will need to progress until they make it inside the house without dying. Find a safe route to the house, and then restart. If you die, you can always try again. Do not worry about getting this on the first attempt. In fact, trying to get this early may cause frustration and ruin the experience. Attempt this after completing the game.
Half hat

This will trigger after collecting half of the game’s hats.

Once you finish the game, start a new game and put on the fedora hat.
In The Palm Of My Hand
Hold tight and we’ll be just fine
Players can hold the hand of their traveling companion. Hold their hand whenever possible to lead them around. Once a certain amount of time has passed, this will trigger upon letting go of their hand. This time is cumulative, so they can freely hold and release to build towards this achievement/trophy.
Toys Are For Kids
There, you’ll be much warmer now
Wear the bear head hat in chapter 3. When you throw the stuffed toy into the incinerator, this will trigger.
Popcorn
Movie night on the ward – all taken care of!
Players will find some corn next to a wheelchair in the same chapter. Throw this corn into the incinerator and then look at it for this to trigger.
Hunger
Plenty of snacks for a growing boy
Players will find a vending machine in chapter 3. Interact with this machine for it to dispense something. Keep using the vending machine for this to trigger.
X Best Friends
I see through you…
In chapter 3, you will find a curtain with a switch to its right. Pull this switch and then hold the hand of your traveling companion. Pull them behind the curtain so it looks like two skeletons holding hands. This will trigger the achievement.
…And Stay Dead!
Better to be safe than sorry
After killing one of the hands in chapter 3 with a sledgehammer. Hit it once more while it is dead for this to trigger. If the trophy/achievement does not trigger, wait a couple of seconds while the hand is motionless, then hit it again.
Objection
Some holes are more picky than others
After the part of chapter 3 where players escape from the mannequins through a vent. Players will keep heading left until they come to a room with a toilet and some cheese. Pick up the cheese and head through the door on the left. Players will see a gate above them. There is an opening on the right side of this gate. Go through this opening and then run while pressed up against the left wall. Players will enter a room with a hole in the floor. Throw the cheese in this hole for this to trigger—and for the hole to make some unsettling noises.
First do no harm
It’s crueller to let him live
After the central antagonist of chapter 3 is trapped in the incinerator, do not pull the lever to activate it. Instead, just continue to progress through the game, and this will trigger.
Sick Kids
Nothing of us remains in the hospital
Shine your flashlight around all the corners of the map as you progress, and you will easily encounter 3 of the glitched remains. The final one is in the same room as doing the Objection trophy/achievement.
Post Industrial
You punctuated someone’s day. Maybe they’ll notice!
In chapter 4, after falling into the room filled with mail. Put on the postal service hat and go to the suitcase on the right side. Take the package from inside this suitcase. Continue to the next room to find a mail chute to dump the package and trigger this achievement/trophy.
Unladylike
Hello, have we met?
In chapter 4, after using the zipline, attempt to backtrack, and you will see a window above the one you climb out of. Go through this window, and you will find a small statue. Break this statue for this to trigger.
Pale Kids
Nothing of us remains in the pale city
Encounter all 4 glitched remains in chapter 4 for this to trigger.
Referee!
Unorthodox, but they all count!
In chapter 2, put on the soccer ball hat and run into the goal.
Mono Tones
My, what a curious song you play!
In chapter 2, run around on the piano keys. It can take a while, but just keep playing the piano this way for this to trigger.
Bully of Bullies
Creative, efficient, brutal. Top of the class!
You will need to sneak through a classroom in chapter 2. After this classroom is a bookshelf that can come crashing down. Players will need to use this bookshelf to fall on the enemies that will chase them from the classroom. Once this bookshelf falls on them, this will trigger.
Fly Free
Rebellion at its finest!
You will find a paper airplane in chapter 2. It can be found among the many beds and a toy ball. Throw this paper airplane out the window for this to trigger.
True Colours
You never know when you need protection from the elements
This trophy/achievement will be obtained by progressing through the story. This makes it an unmissable trophy/achievement.
School Kids
Nothing of us remains at the school
Encounter all 6 glitched remains in chapter 2 for this to trigger.
Fair Prey
It’s not murder if they’re bad people… is it?
Put on the hat found in the house of chapter 1. Keep it on while using the shotgun to kill a pursuing enemy for this trophy/achievement to trigger.
Wild Kids
Nothing of us remains in the wilderness
Encounter all 4 glitched remains in chapter 1 for this to trigger.
Twenty-Six
Relax, she heard you!
Call out to your traveling companion 26 times over the course of the game’s chapters. Calling them periodically will cause this to trigger.
Medicine ball
Any other last requests?
In chapter 3, various hands reach for the player through a door. Players will find a ball in the room. Pick up this ball and throw it at the hands for this to trigger.

Merciful feat

In chapter 2, players will come to a room with an enemy drawing on the floor. The enemy will have a limited range before they fall over. Players will need to run and grab the metal stick when they do. If the player kills this enemy with the stick, they will need to start this scene over. Players will need to use this stick to leave the area without killing this enemy for this achievement/trophy to trigger.
